基底节区Ⅲ级血管周围间隙扩大的MR诊断

摘　　要：目的分析基底节区Ⅲ级血管周围间隙扩大的MR I表现,探讨MR I的诊断优势及价值,提高对其诊断及鉴别诊断的水平。资料与方法回顾性分析34例基底节区Ⅲ级血管周围间隙扩大的MR I平扫及增强、磁共振血管造影(MRA)、扩散加权成像(DW I)表现,比较其中12例CT、9例MR随访及对照组表现。结果 34例中,28例单发,共44个病灶,其中前联合24个,豆状核8个,外囊4个,丘脑4个,内囊2个,苍白球2个;直径均>3 mm,边界清楚,3例呈簇状聚集,其余孤立分布;病灶均呈长T1、长T2信号,液体衰减反转恢复序列(FLAIR)35个病灶呈完全低信号,9个边缘呈环状高信号;7例DW I无异常;4例合并脑萎缩;9例MRA提示动脉硬化;12例CT均表现为囊状水样低密度,体积小于MR I测量值;9例随访无明显变化,对照组信号无差异。结论 MR I对基底节区Ⅲ级血管周围间隙扩大的诊断及鉴别诊断具有明显的优势及实用价值。Objective To analyze the MR features of the basal ganglia＇s Virchow-Robin space grade Ⅲ expansion in order to improve its diagnostic accuracy.Materials and Methods Plain and enhanced MR,MRA,DWI findings of 34 cases with the basal ganglia＇s Virchow-Robin space grade Ⅲ expansion were analyzed retrospectively,and compared with the findings of CT in 12 cases,9 cases of MR follow-up and the control group.Results Among 34 patients,28 patients were single,a total of 44 lesions were found：24 in the former joint,8 in Wilson,4 in external capsule,4 in the hypothalamus,2 in internal capsule,2 in globus pallidus;diameter of all lesions was greater than 3 mm with clear boundary,and 3 cases clustered together,the remains were isolated.All lesions had long T1,T2 signal,35 lesions had hypointensity signal completely in FLAIR,9 lesions with edge ring-like hyperintensity signal on FLAIR;DWI without abnormality was found in 7 cases;4 cases with cerebral atrophy;9 cases with atherosclerosis indicated by MRA;12 lesions were depicted as saccular water density in CT and smaller than MR measurements.9 patients were followed up without significant changes.The control group had normal intensity signal.Conclusion MRI has greater value in the diagnosis and differential diagnosis of basal ganglia＇s Virchow-Robin space grade Ⅲ expansion.
