少见部位骨嗜酸性肉芽肿的影像学诊断

摘　　要：目的探讨少见部位骨嗜酸性肉芽肿(eosinophilic granuloma of bones,EGB)的影像学表现及鉴别诊断。资料与方法回顾性分析经病理证实的10例少见部位EGB患者的X线平片、CT及MRI表现。结果 10例中,单发8例,多发2例。其中,四肢骨6例,锁骨、肩胛骨各1例,骨盆2例。四肢骨主要表现为溶骨性破坏,边界清晰,轻度硬化边,骨皮质受压变薄呈"弧"形压迹或"扇"形改变,可伴有骨膜反应,多数连续,超过病变长度;其他少见部位表现类似恶性骨肿瘤,缺乏特异性。结论四肢骨EGB现有一定的特征,综合分析可提高本病诊断及鉴别诊断水平,其他少见部位EGB诊断困难,需与恶性骨肿瘤鉴别。Objective To discuss Imaging appearance and differential diagnosis of eosinophilic granuloma of bone in the rare site.Materials and Methods Plainflim,CT and MRI manifestations of ten cases with eosinophilic granuloma of bone in the rare site,confirmed by pathology in our hospital from,2005 to 2010,were analyzed retrospectively.Resu-lts There were 8 cases with single lesion and the others with multiple lesions.Among the ten cases,six cases were located in extremity bone,one case respectively in clavicle and scapula,two cases in the pelvis.The lesions in limbs showed well defined osteolytic destruction,mild sclerosis margin with thinning of cortical bone compression curved notch or fan change,maybe associate with continuous periosteal reaction more than lesion＇s length.The performance in other rare sites was similar to malignant bone tumors,lack of specificity.Conclusion The imaging findings of eosinophilic granuloma of limbs have certain characteristics,a comprehensive analysis can improve the diagnosis and differential diagnosis of disease,other rare eosinophilic granuloma of bone is difficult to diagnose,needs differ from malignant bone tumors.
