摘 要:目的探讨数字化体层融合(digital tomosynthesis,DTS)在膝关节病变检查中的应用价值。资料与方法30例膝关节病变患者,均先后行X线平片、DTS和CT检查。将上述影像资料传至工作站,由2名具有3年以上影像诊断经验的医师分别进行双盲阅片,判断有无病变及其影像特征,并做出相应诊断,意见不致时,由2位医师再共同阅片,以取得一致结果。以CT结果作为标准,分别判断X线平片、DTS发现病变的影像特征的敏感性。采用配对四格表资料的χ2检验比较两者的差异性。结果 X线平片表现:7例阴性,23例阳性,共发现99个影像特征。DTS表现:3例阴性,27例阳性,共发现116个影像特征。CT表现:共发现123个影像特征。以CT为标准对照,X线平片检查膝关节病变影像特征的敏感性为80.5%(99/123),DTS的敏感性为94.3%(116/123),两者差异有统计学意义(χ2=10.67,P<0.05)。结论 DTS可显著提高膝关节病变影像特征的敏感性,可以作为膝关节病变X线平片检查的良好补充。Objective To discuss the value of digital tomosynthesis in the diseases of Knee Joint. Materials and Methods Thirty patients suspected of having the diseases of knee joint received knee joint radiography,digital tomosynthesis and CT examination. Imaging data were transferred to work station and were reviewed by 2 experienced radiologists in a double blind method. The number, location and sort of characteristics of images were recorded. Then,2 radiologists reviewed the all images again,and discussed in consensus. The sensitivity of knee joint radiography and digital tomosynthesis for detection of eharafteristies of images were respectively calculated according to the CT results. Chi square test was used for radiography,digital tomosynthesis and CT examination. Results Of 30 patients,23 were positive by X-ray radiography and 7 were negative,the total number of 99 characteristics of images was detected,while 116 characteristics of images in 27 patients were detected by digital tomosynthesis,and only 3 patients were negative. CT demonstrated 123 characteristics of images in 30 patients. Using the result of CT as the gold standard,the sensitivity of X-ray radiography and digital tomosynthesis was 80.5% (99/123) and 94.3% ( 116/123 ) ( x^2= 10.67, P 〈 0.05 ) ,respectively. Conclusion Digital tomosynthesis has a high sensitivity tor detection of Knee joint diseases, and could be an excellent supplementary technique to X-ray radiography.
