乌司他丁对急性肺损伤/急性呼吸窘迫综合征治疗效果分析

摘　　要：目的:探讨乌司他丁(ulinastatin,UTI)对急性肺损伤/急性呼吸窘迫综合征(acute lung injury/acute respiratory distress,ALI/ARDS)的治疗效果。方法:ALI/ARDS患者40例,随机分为UTI治疗组(n=20)和对照组(n=20);对照组接受常规治疗,UTI治疗组在常规治疗基础上静脉推注乌司他丁20万u,每8h 1次,连用10天。观察两组的临床疗效、治疗前后心率、呼吸、、动脉血气以及血清肿瘤坏死因子-α(TNF-α)、IL-6水平变化。结果:治疗组有效率90.0%,明显高于对照组65.0%(X^2=3.856,P<0.05);两组治疗前后各项指标比较均有统计学差异(t=2.231、2.311、2.215、2.301、2.315、2.333、2.341、2.401、2.451、2.342、2.332、2.333,P均<0.05);两组治疗8d,Pa0_2、心率、呼吸、有显著性差异(t=2.232、2.311、2.3415,P均<0.05);两组治疗前后TNF-α、IL-6含量差异有统计学意义(t=2.378、2.431、2.331、2.322,P均<0.05),结论:乌司他丁减轻炎性反应对机体的损伤,改善ALI/ARDS的预后。Objective To investigate the curative effect of ulinastatin on acute lung injury/acute respiratory distress syndrome（ALI/ARDS）.Methods 40 patients with ALI/ARDS were randomly divided into treatment group （n=20）and control group （n= 20）.The control group received only the routine therapy, and the treatment group were given ulinastatin（200 ku intravenously, once every 8 hours, and continued for 10 days） in addition to the routine treatment.The Methods：Thirty-four patients with ALI/ARDSwere randomly divided into treatment group（n=17） and control group （n=17）. The control group received only the routine therapy, and the treatment group were given ulinastatin （200 ku intravenously,once every 8 hours, and continued for 10 days） in addition to the routine treatment.The index of clinical efficacy,heart rate,breathing, arterial blood gas,serum tumor necrosis factor-or （TNF-a）.Interleukin -6 （IL-6） before treatment and after treatment were observed.Results The effet rate 90.0% in treatment group were significantly higher than 65.0% in control group （X2=3.856,P〈0.05）;The projects Comparative before and after treatment in both groups were statistically significant （t=2.231,2.311,2.215,2.301,2.315.2.333,2.341,2.401,2.451,2.342,2.332,2.333, all P〈0.05）; The index of PaO2.heart rate, breathing in two groups were significant difference（t=2.232,2.31 1,2.3415,all P〈0.05）.Conclusion Ulinastatin may prevent the systemic injury by inflammatory response and improve the prognosis of ALI/ARDS.
