摘 要:目的:探讨彩色多普勒超声(CDFI)在放疗所致颈动脉损伤中的诊治价值。方法:采用CDFI检查17例头颈部恶性肿瘤患者放疗后的颈动脉,并对行经皮颈动脉成形加血管内支架放置术(CAAS)治疗的重度及完全闭塞患者的疗效进行评价。结果:17例患者中,3例左锁骨下动脉重度狭窄;1例右锁骨下动脉远端闭塞;8例一侧或双侧颈内动脉中度以上狭窄,其中重度者3例;5例一侧或双侧颈总动脉中度以上狭窄,其中重度者2例。病变均位于照射野内。9例重度狭窄或闭塞的患者在DSA确诊后接受CAAS治疗。治疗后超声随访,放疗性动脉损伤病变为临床治愈。结论:CDFI是诊断放疗所致颈动脉损伤的有效手段,并对术后疗效评估提供了可靠的方法。Objective:To evaluate the value of color Doppler flow imaging(CDFI) in the diagnosis and treatment of radiation-induced carotid artery injury.Methods:After radiotherapy,17 patients with head and neck cancer were performed CDFI of carotid,which was used to evaluate the efficacy of carotid artery angioplasty and stent(CAAS) with carotid artery severe stenosis or occlusion.Results:Among 17 patients,left subclavian artery severe stenosis in 3 cases,right subclavian artery occlusion in 1 case,moderate or severe internal carotid artery(unilateral and bilateral) stenosis in 8 cases among which 3 cases were severe stenosis,moderate or severe common carotid artery(unilateral and bilateral) stenosis in 5 cases,among which 2 cases were severe stenosis.All arterial lesion limited to the radiation field.9 patients with severe stenosis or occlusion lesions were performed CAAS.The arterial lesions were considered to be cured clinically after follow-up by CDFI.Conclusion:CDFI is one of effective means in diagnosis of radiation-induced carotid artery injury,and it provides a reliable method to assess the curative effect.
