摘　　要：目的:运用组织速度图分析不同病因引起的慢性心衰患者心脏机械运动时间间期特点。方法:选择30例健康志愿者作为对照组,慢性心力衰竭的患者60例作为病例组,根据病因分为缺血性心肌病组(ICM组)、扩张性心肌病(DCM组)(每组30例)。在静息状态下获取标准心尖二腔、四腔、长轴观的二维及彩色组织多普勒图像,脱机分析、测量左心室各室壁壁基底段、中间段时间间期。结果:与对照组比较,心衰组的等容收缩时间、等容舒张时间、心房收缩时间显著延长,射血时间、减速射血时间、快速充盈时间缩短;对照组、DCM组同一室壁基底段与中间段时间间期参数无明显差异,ICM组左室前壁、下壁、前间隔、后壁的射血时间基底段明显高于中间段。结论:心力衰竭患者等容收缩时间、等容舒张时间和心房收缩时间延长,射血时间、减速射血时间和快速充盈时间缩短;射血时间、减速射血时间对扩张型心肌病和缺血性心肌病引起的慢性心衰具有一定的鉴别诊断价值。Objective：To quantitatively assess the myocardial motion time intervals in the patients with heart failure from different cause by quantitative tissue velocity imaging（QTVI）.Methods：The study include 60 patients with chronic heart failure who were compared with 30 healthy subjects,the patients were divided into two groups： group ICM（ischemic cardiomyopathy） and group DCM（dilated cardiomyopathy）.At resting condition,two-dimensional tissue Doppler images of standardized apical,two-chamber,four-chamber and long-axis view were taken with both groups by GE VIVID7 machine,cardiac intervals were measured in basal and middle segments.Results：We found that patients in comparison with controls IVCT,IVRT and AST were prolonged,the ET,SET and RFT were shortened,in heart failure group.The time intervals parameters in the different segments of the same ventricular wall have no significant difference in group control and group DCM,but in the group ICM,the ET had difference between basal and middle segments in anterior wall inferior wall,anterior septum and posterior wall.The ET in basal segments was longer than in middle segments.Conclusion：① IVCT,IVRT and AST were prolonged,the ET,SET and RFT are shortened,in heart failure patients;② ET,SET have shown certain value in differential diagnosis of ICM and DCM.
