心肌肌钙蛋白T检测对维持性血透尿毒症患者心血管事件发生的临床意义

摘　　要：目的探讨检测心肌肌钙蛋白T(cTnT)对维持性血透的尿毒症患者心血管事件发生的临床意义。方法测定136例维持性血透尿毒症患者透析前、后的cTnT,每6个月测定cTnT1次,随访3年,观察患者心血管事件及死亡事件发生情况,分析其与cTnT的关系。同时测定100名健康体检者的cTnT,并作对比分析。结果维持性血透尿毒症患者组cTnT水平与对照组比较差异有统计学意义(P<0.05),患者中cTnT升高组心血管事件发生、心血管事件死亡率、总死亡率与cTnT正常组比较差异有统计学意义(P<0.05),cTnT与患者危险死亡度显著相关(r=0.52,P<0.05)。尿毒症患者组cTnT阳性率为30%。结论 cTnT是维持性血透尿毒症患者心血管事件发生的危险因素,与患者的预后密切相关,cTnT的检测有助于维持性血透患者心血管并发症的早期诊断。Objective To investigate the clinical significance on the determination of cardiac troponin T（cTnT） for cardiovascular events in uremic patients undergoing hemodialysis.Methods The cTnT was determined in 136 uremic patients undergoing maintaining hemodialysis before and after dialysis every 6 months.The cardiovascular events and death events were recorded during the follow-up period for 3 years,and the relationship with cTnT was analyzed.The cTnT of 100 healthy subjects were determined,and the results were comparatively analyzed.Results The differences of cTnT levels had statistical significance between in the uremic patients and in controls（P0.05）.The total mortality rate,cardiovascular mortality rate and occurrence of cardiovascular events in the cTnT increasing group were significant with those in the normal cTnT group（P0.05）.The cTnT was correlated with the mortality danger（r=0.52,P0.05）.The positive rate of cTnT in uremic patients was 30%.Conclusions In uremic patients undergoing hemodialysis,the cTnT is a risk factor for cardiovascular events and is related to the prognosis.The determination of cTnT is helpful for early diagnosis of cardiovascular events in uremic patients undergoing hemodialysis.
