应用ROC曲线评价产前IgG抗A(B)效价检测对早期诊断ABO新生儿溶血病的价值

摘　　要：目的研究O型孕妇产前血清IgG抗A(B)效价对早期诊断ABO新生儿溶血病(HDN)的价值。方法采用微柱凝胶法检测704例夫妻血型不同的O型孕妇产前血清IgG抗A(B)效价,用红细胞(RBC)直接抗人球蛋白试验、RBC抗体放散试验、血清游离抗体测定等3项试验确定患儿是否患有HDN,用受试者工作特性(ROC)曲线进行分析评价。结果孕妇产前血清IgG抗A(B)效价升高,HDN的发生率也增加,HDN患儿母亲产前血清IgG抗A(B)的ROC曲线下面积为0.869 2。结论孕妇产前血清IgG抗A(B)效价测定对早期诊断HDN有一定的价值,建议以1∶128为诊断临界点。Objective To study value of the O type prenatal serum IgG anti-A（B） titers for the early diagnosis of ABO hemolytic disease of newborn（HDN）.Methods O type prenatal serum IgG anti-A（B） titers of 704 couples with different blood groups were determined by the microcolumn gel test.By red blood cell（RBC） direct antiglobulin test,RBC antibody elution test and serum free antibody assay,the newborns were diagnosed with HDN or not.By receiver operating characteristic（ROC） curve,the results were analyzed and evaluated.Results When prenatal serum IgG anti-A（B） titers increased,the incidence rate of HDN was also increased.In prenatal serum IgG anti-A（B） mothers with HDN newborns,the area under ROC curve was 0.869 2.Conclusions Prenatal serum IgG anti-A（B） titer determination is valuable for the early diagnosis of HDN,and it is recommended to 1∶128 for the diagnosis critical point.
