广州地区心血管病患者危险因素与血脂水平的分析

摘　　要：目的探讨广州地区近5年来心血管病患者部分相关危险因素对血脂异常的影响。方法采用现状研究方法,分析1 894例心血管病住院患者不同危险因素与血脂水平的关系。结果 (1)不同性别、年龄段患者的血脂水平均存在差异,主要表现为女性患者总胆固醇(Tch)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)均显著高于男性患者。(2)多因素回归分析表明,各项血脂异常存在不同危险因素的影响,其中性别、年龄、血糖和尿酸值是重要因素,在4项血脂异常的回归模型中有不同程度的作用。(3)筛选变量后的单因素分析显示,血脂变化趋势随年龄增长而下降,其中男性Tch、TG和LDL-C的趋势检验有统计学意义;高尿酸血症(HUA)对患者血脂异常的影响较为突出,男性HUA组患者各项血脂指标与对照组差异有显著性,女性患者中仅三酰甘油(TG)值一项HUA组显著高于对照组;糖尿病对男女患者TG水平的升高和HDL-C水平的降低有显著影响。结论不同年龄、性别患者血脂水平及血脂异常的危险因素有一定差异。血脂异常的防治应采取更具针对性的措施。Objective To investigate the effects of several risk factors on dyslipidemia among patients with cardiovascular disease in Guangzhou during recent years. Methods Cross-sectional study was performed to analyze the correlation between plasma lipid levels and some risk factors among 1 894 hospitalized patients in the Department of Cardiology of Zhujiang Hospital. Results（1） Significant differences in plasma lipid levels existed among patients with different genders and age.Especially,the Tch,LDL-C and HDL-C levels in female patients were signi ficantly higher than those in males.（2） Logistic multivariate regression analysis showed that 4 important risk factors,including gender,age,Glu and UA,respectively affected on all four plasma lipid parameters.（3） Tch,TG and LDL-C changing trends were significantly weakened with increasing age.Significant differences were revealed in all the 4 lipids levels between male patients with HUA and male controls,while only significant difference in TG was revealed between female patients and controls.Simultaneously,significant increase in TG and reduction in HLD-C were revealed in diabetic patients. Conclusion Diverse risk factors of dyslipidemia are recognized in different population,which advocate specific preventive and treatment protocol.
