摘  要:目的:评价脂肪抑制的质子加权快速自旋回波(FS-PDWI-FSE)序列在诊断膝关节前、后交叉韧带(ACL/PCL)损伤中的应用价值。方法:回顾性分析2009-10—2010-04间的31例膝关节外伤后行MR检查的病例。在接受关节镜检查前所有的病例均行PDWI-PSE、T2WI-PSE、PS-PDWI-FSE、PS-T2WI-FSE序列矢状面扫描。分析4种不同序列在ACL/PCL损伤中的表现,并比较不同扫描序列对交叉韧带损伤诊断的敏感性、特异性和准确率。结果:与关节镜相比,所有的4种序列对31例患者中交叉韧带撕裂显示的敏感性、特异性和准确率分别是86%、98%和94%;对ACL撕裂显示的敏感性、特异性和准确率分别是84%、100%和90%。有2例PCL在所有扫描序列中显示为真阳性,1例完整的PCL误认为撕裂被诊断为假阳性。结论:FS-PDWI/T2WI FSE序列在对ACL/PCL损伤的检查中明显优于PDWI/T2WIFSE序列。Objective To evaluate fat-suppressed(FS) proton-density-weighted(PDW) turbo spin-echo(FSE) magnetic resonance imaging for the detection of anterior and posterior cruciate ligament lesions.Methods In a retrospect study,31 knee joints were imaged on a MR scanner between October 2009 to April 2010.Prior to arthroscopy using following sequences: FS-PDW/T2W FSE;PDW/T2W FSE.Anterior and posterior cruciate ligaments(ACL/PCL) were evaluated.Sensitivity,specificity and accuracy were calculated.Results For all four sequences in the 31 patients compared with arthroscopy,the correlation sensitivity,specificity and accuracy were 86%,98% and 94% for detection,and 84%,100% and 90% for ACL-ruptures respectively.The two PCL-ruptures were true positive in all sequences,but one intact PCL was diagnosed as torn(false positive).Conclusion FS-PDW/T2W FSE is comparable nakedness to PDW/T2W FSE for the detection of ACL/PCL-lesions.
