腹腔镜经腹腔途径离断性肾盂成形术疗效再评估

摘　　要：目的:探讨腹腔镜经腹腔途径离断性肾盂成形术治疗肾盂输尿管连接部梗阻(UPJO)的技术特点、可操作性、并发症的防范处理等,充分评估该术式的临床适用性。方法:采用腹腔镜经腹腔途径离断性肾盂成形术治疗UPJO 23例,B超检查重度积水9例,中度12例,轻度1例,合并肾盂结石1例。结果:23例手术均获成功,手术时间90～150 min,术中失血20～100 ml,平均60 ml。术后住院时间5～7天,平均6天。术后无吻合口狭窄、漏尿及其他并发症发生。随访6～30个月,所有患者肾功能明显改善,肾积水消失或减轻。结论:针对UPJO,经腹腔途径离断性肾盂成形术有自身不可替代的特点和优点,仍是治疗UPJO有效的手术方式。Objective：To explore the technical feature, rationality and prevention of complications of transperitoneal laparoscopic dismembered pyeloureteroplasty for ureteropelvic junction obstruction. Methods： 23 cases of ureteropelvic junction obstruction were operated with transperitoneal laparoscopic dismembered pyeloureteroplasty, and male 16 and female7, age （25-46）, ]5 on the left side and 8 right side. In these 23 cases, 9 severe, 12 midrange and 1 mild hydronephrosis and 1 pelvis stone were founded. Results： 23 operations were all completed suc cessfully, and operative time is （90 150 rains）, blood loose （20 100 ml）, length of stay （5-7days）. No stricture, leakage and other complications occurred in （6-30 mons） follow up, and all symptoms disappear. Conclusions： Transperitoneal laparoscopic dismembered pyeloureteroplasty is an important and effective method for ureteropelvic junction obstruction with Irreplaceable characteristics compared with retroperitoneal laparoscopie dismembered pyeloureteroplasty.
