长沙市102家医院2003—2009年住院患者抗菌药物应用分析

摘　　要：目的分析长沙市102家医院2003—2009年城镇职工基本医疗保险参保住院患者抗菌药物用药构成及费用的变化趋势。方法应用统计描述和单因素分析方法分析2003—2009年长沙市102家医院24.02万住院患者的抗菌药物使用情况。结果 2003—2009年间抗菌药物使用率为70.07%,并呈逐年下降趋势,从2003年的78.96%下降至2009年的66.26%;抗生素类抗菌药物使用率为64.40%(头孢菌素类抗生素为45.72%,青霉素类抗生素为25.48%),人工合成抗菌药物使用率为36.57%。7年间抗菌药物使用率前3名为左氧氟沙星(13.34%)、头孢噻肟(8.14%)和头孢哌酮-舒巴坦(8.03%);抗菌药物用药金额排名前10位的药物中头孢菌素类占据8种。使用了抗菌药物的患者人均抗菌药物费用为1 705.89元,占住院费用的17.53%,占西药费用的41.17%。结论长沙市医保住院患者使用抗菌药物的比例有下降趋势,但仍然偏高,有必要进一步加强抗菌药物的合理使用。AIM To assess the constitution and expense trends of antibacterial drugs of inpatients in basic medical insurance in 102 Changsha hospitals from 2003-2009.METHODS The use of antibacterial drugs of 240.2 thousand inpatients in 102 Changsha hospitals from 2003-2009 were analyzed statistically in term of description and single-factor analysis.RESULTS The total prescription rate of antibacterial drugs was 70.07% during 2003-2009,and showed a declining trend from 78.96%（in 2003） to 66.26%（in 2009）.The prescription rate was 64.40%for antibiotics drugs（45.72%for cephalosporins and 25.48%for penicillins） and 36.57%for synthetic antibacterial drugs.The three most frequently used antibacterial drugs were levofloxacin （13.34%）,cefotaxime（8.14%）,and cefoperazone/sulbactam（8.03%）。The cephalosporins drugs possessed eight in the top ten drugs in consumption sum.The per capita cost of antibacterial drugs was 1 705.89 yuan, accounting for 17.53%of the hospitalization cost and 41.17%of the medicine expense.CONCLUSION The prescription rate of antibacterial drugs of inpatients in basic medical insurance shows a decreasing trend in Changsha,but still in a high level,so it is necessary to further strengthen the rational use of antibacterial drugs.
