摘  要:目的应用VVI定量评价胰岛素治疗后糖尿病心肌病大鼠心肌功能的变化。方法 30只雄性SD大鼠,随机分为糖尿病组(DM组)、胰岛素治疗组(INS组)和正常对照组(CON组),每组10只。腹腔注射链脲佐菌素建立糖尿病模型后,INS组给予胰岛素皮下注射(6 U/kg体质量),每日一次。于造模成功12周后开胸,取左心室短轴图像进行VVI分析,并取心肌组织行透射电镜观察。结果与DM组比较,INS组收缩和舒张期径向峰值速度(Vs、Vd)、径向峰值应变(Sr)、收缩期径向峰值应变率(收缩期SRr)以及切向峰值应变(Sc)、收缩和舒张期切向峰值应变率(收缩期SRc、舒张期SRc)均较高(P<0.05);舒张期径向峰值应变率(舒张期SRr)与DM组差异无统计学意义(P>0.05)。与CON组比较,INS组除舒张期SRr、收缩期SRc、舒张期SRc较低(P<0.05)外,余指标与CON组差异无统计学意义(P>0.05)。结论胰岛素能明显改善糖尿病心肌病大鼠的心肌功能;VVI应变率指标在评估糖尿病心肌病治疗疗效及预后方面起着重要作用。Objective To evaluate the change of myocardial function of diabetic cardiomyopathy in rats with insulin treatment using velocity vector imaging(VVI) technology.Methods Thirty rats were divided into 3 groups: Diabetes mellitus group(DM,n=10),insulin-treatment group(INS,n=10) and control group(CON,n=10).DM models were established through intraperitoneal injection of streptozotocin.Rats in INS group were given insulin hypodermic injection at dose of 6 U/kg once a day ever since the DM models were established.After 12 weeks,echocardiography on short axis view was performed on each rat to evaluate the VVI parameter,and the myocardium was collected for transmission electron microscope examination.Results Compared with DM group,systolic and diastolic radial velocity(Vs,Vd),radial strain(Sr),systolic radial strain rate(systolic SRr),circumferential strain(Sc),systolic and diastolic circumferential strain rate(systolic SRc,distolic SRc) in INS group were higher(P〈0.05).Diastolic radial strain rate(diastolic SRr) had no statistical difference with DM group(P〉0.05).Compared with CON group,diastolic SRr,systolic SRc and diastolic SRc were lower in INS group(P〈0.05),other parameters had no statistical difference between the two groups(P〉0.05).Conclusion Insulin could significantly improve the myocardial function of diabetic cardiomyopathy in rats.VVI strain rate plays an important role on evaluating the curative effect and prognosis in diabetic cardiomyopathy.
