全乳晕入路与胸乳入路腔镜双侧甲状腺手术的比较

摘　　要：目的比较全乳晕入路与胸乳入路腔镜双侧甲状腺手术的临床效果。方法 57例双侧甲状腺病变患者分别采用全乳晕入路(21例)与胸乳入路(36例)行腔镜下双侧甲状腺手术,比较两种入路手术的临床治疗效果。结果全乳晕入路与胸乳入路腔镜下双侧甲状腺手术在手术时间、术后住院时间、术中出血量、术后第一天引流量之间无显著差异(P>0.05),但是全乳晕入路美容效果优于胸乳入路。结论全乳晕入路腔镜甲状腺手术安全可行,有近乎完美的美容效果,但是开展初期宜慎重。Obective To compare the clinical efficacy of breast approach and areola of breast approach in endoscopic thyroidectomy.Methods Fifty-seven patients with bilateral nodular goiter were divided into areola of breast approach group（n=21） and breast approach group（n=36）.The therapeutic efficacy was compared between the two groups.Results There was no significant difference in operating time,postoperative hospital duration,intraoperative blood loss and postoperative drainage volume at the first day between the two groups（P0.05）,but better cosmetic outcomes were achieved in areola of breast approach group.Conclusion Endoscopic thyroidectomy through areola of breast approach is safe and feasible and approximately perfect cosmetic outcomes can be achieved,however,more attention should be paid at the initial stage.
