超声刀在腹腔镜胆囊切除术的应用

摘　　要：目的 探讨胆囊结石合并肝硬化的患者使用超声刀行腹腔镜下胆囊切除术中的应用价值.方法 对56例合并肝硬化的胆囊结石患者在腹腔镜下使用超声刀行胆囊切除术的资料进行回顾性分析.结果 56例均成功完成胆囊切除术,无中转开腹;平均手术时间46min,术中平均出血15ml,无术后出血、胆管损伤、胆漏、腹腔感染、肝功能衰竭等并发症.术后平均住院4d,均顺利出院.结论 腹腔镜下应用超声刀对合并肝硬化的胆囊结石患者行胆囊切除术具有止血彻底、出血少、肝功能损害小、并发症少、操作简单容易掌握等优点,是治疗胆囊结石症有效、安全的好方法.Objectives To explore the application value of Uhracision harmonic scapel （uhs） in laparoscopic cholecystectomy （LC） in gallstone patients with liver cirrhosis. Methods The data of 56 patients with cholecystolithiasis combined cirrhosis, who underwent laparoscopic cholecysteetomy by use of Uhracision harmonic scapel,were analysed retrospectively. Results LC were successfully in all 56cases, there were no conversion surgery and severe complications such as bile leakage, postoperative hemorrhage, bile duct injury, abdominal infection and hepatic failure. The average operation time was 46 min ,the average intraoperative blood loss was 15ml, the average postopreative hospital stay was 4 d, all patients recovered and were discharged. Conclusion UHS can be applied in LC tor gallstone patients with liver cirrhosis with advantages including Completive hemostasis, little blood loss, tiny injury, mild liver damage and little complications. So it is a good for LC.
