17例伪马勒伯克霍尔德菌检测与耐药性分析

摘　　要：目的了解伪马勒伯克霍尔德菌病的发病特点和耐药情况。方法采用Vitek-32全自动细菌分析系统对海南省人民医院住院确诊治疗的伪马勒伯克霍尔德菌感染者17例进行细菌鉴定及药敏试验。结果患者多见于男性,以农民发病最多,临床表现均有畏寒或寒战、发热。伪马勒伯克霍尔德菌对抗菌药物具有多重耐药性,对氨苄西林、头孢唑啉、头孢西丁、头孢呋辛钠、头孢呋辛酯、丁胺卡那霉素、头孢吡肟、庆大霉素均100.0%耐药,左旋氧氟沙星23.5%和头孢噻肟17.6%,未发现对亚胺培南、哌拉西林/他唑巴坦和美洛培南耐药。结论伪马勒伯克霍尔德菌感染临床表现无特殊性,误诊率高,确诊依靠细菌学检测。亚胺培南、哌拉西林/他唑巴坦和美洛培南对伪马勒伯克霍尔德菌敏感,应作为治疗伪马勒伯克霍尔德菌病感染的首选用药。Aim To analyze drug-resistance of Burkholderia pseudomalleiin 17 cases.Methods Burkholderia pseudomallei were detected from 17 patients diagnosised as melioidosis in Hainan Provincial People＇s Hospital between April 2009 and April 2010,and drug resistance tests were conducted.Results Mst patients were male farmers with clinical manifestations of fever and shiver.The resistance of Burkholderia pseudomallei to ampicillin,cefazolin,cefoxitin,cefuroxime sodium,cefuroxime axetil,amikacin,cefepime,gentamycin were 100%,the resistance to levofloxacin,cefotaxime 23.5% and 17.6%,while it was sensitive to imipenem,piperacillin-tazobactam,meraopenem,the sensitivity to cefoperazone-sulbactam was 88.2% and 76.5% to ceftazidime.Conclusion The misdiagnosis rate of Burkholderia pseudomallei infection is high and confirmed diagnosis depends on pathogenic detection.agnosis by investigations on bacteria.Imipenem,piperacillin-tazobactam,meraopenem are the drugs of choice for treatment of Burkholderia pseudomallei infection.
