摘 要:目的观察静脉或硬膜外注射舒芬太尼对福尔马林致痛家兔外周血 β-内啡肽(13-EP)和氧自由基(OFR)表达的影响。方法选择18只健康新西兰大白兔随机均分为三组(n=6):静脉镇痛组(A组)、硬膜外镇痛组(B组)、生理盐水对照组(C组)。每组家兔均予足底注射5%福尔马林0.5ml致痛,致痛后A组和B组分别经静脉和硬膜外注射舒芬太尼0.46 βg/kg和O.64 βg/kg,C组静脉注射等容量生理盐水。记录注药后5、10、15、20、25、30、35、40、45min家免的疼痛评分;检测致痛前(To)、注药后10、20、40min、2、8、24和48h(T1~T7)血清中 β-EP含量;根据血清中丙二醛(MDA)含量评估OFR水平。结果与To时比较, β-EP含量在A、B组R~rr7时、C组T1~T7时均升高(P〈O.05),MDA含量在T1~T7时A、B组降低,C组升高(P〈o.05);与C组注药后相应时点比较,A.B组疼痛评分,血清伊EP和MDA含量均低于c组(P〈o。05)。结论静脉或硬膜外注射舒芬太尼可有效抑制福尔马林致痛家兔 β-EP含量和OFR表达。Objective To observe the influence of intravenous or epidural analgesia with sufentanil on β-endorphin ( β-EP) and oxygen free radical (OFR) expression in rabbits with formalin-induced pain. Methods Eighteen healthy rabbits were equally randomized into three groups (n= 6): intravenous analgesia group (group A), epidural analgesia group (group B) and control group (group C). All rabbits were injected with 5% formal 0. 5 ml to establish formalin-induced pain model. Then sufentanil 0. 46 μg/kg and 0. 64/,g/kg were intravenously and epidurally injected in group A and group B, respectively. The same volume of saline was intravenously injected in group C as control. Pain scores were recorded every 5 rain from 5 rain after administration to 45 rain after administration The level of ffEP was detected before formalin injection (To)and at 10 min, 20 min, 40 min, 2, 8, 24 h and 48 h (T1-T7) after sufentanil or saline injection, and the level of OFR was evaluated through the level of MDA. Results Compared with To, the level of β-EP increased at T2-T7 in group A and group B and at T1-T7 in group C (P〈0. 05). The level of MDA at T1-T7 decreased in group A and group B and increased in group C. Compared with group C at the same time points, pain scores,and the levels of β-EP and MDA were lower in group A and group B than in group C (P〈0. 05). Conclusion Intravenous or epidural analgesia with sufentanil may effectively inhibit the expression of β-EP and OFR in rabbits with ormalinindueed pain.
