床旁彩色多普勒超声在ICU重症患者下肢深静脉血栓诊疗中的价值

摘　　要：目的 探讨床旁彩色多普勒超声诊断ICU重症患者下肢深静脉血栓形成中的价值.方法 根据有无下肢深静脉血栓的临床症状将121例ICU重症患者分为A组102例和B组19例,使用床旁彩色多普勒超声分别对两组治疗前后进行检查.结果 A组102例患者中,检出深静脉血栓9例,均位于小腿肌间静脉.经临床抗凝治疗1周后,所有血管完全通畅.B组19例患者中,检出深静脉血栓17例.经溶栓、抗凝治疗1周～1个月后复查,其中4例血管完全通畅,9例部分再通,3条无明显变化,1例有加重.结论 床旁彩色多普勒超声可为ICU重症患者下肢深静脉血栓的诊断提供可靠依据。Objective To explore the value of hand - carried color Doppler ultrasound （CDU） in diagnosis of deep venous thrombosis（DVT） in Intensive Care Unit （ICU）patients. Methods One hundred and twenty - one ICU patients were divided into two groups according to the clinical symptoms of low limb DVT, and they were examined by hand - carried CDU before and after treatment. Results There were 9 cases of DVT in group A without clinical symptoms（9/102）, the thrombus existed in calf muscle veins. After one week of anticoagulation treatment, all the blood vessels became unobstructed. While there were 17 cases of DVT in group B with clinical symptoms （ 17/19 ）. After one week to one month of anticoagulation treatment, 4 cases became unobstructed, 9 cases became partial unobstructed, 3 cases had no changes, and 1 case became serious. Conclusion Hand - carried CDU is an effective method to diagnose low limb DVT in ICU patients, it can provide reliable information and effectively evaluate the treatment effects.
