慢性丙型肝炎患者甲状腺功能异常情况观察

摘　　要：目的了解慢性丙型肝炎患者甲状腺疾病发生情况及其与肝功能、病毒复制、肝硬化发生及合并症的关系。方法检测133例未接受干扰素治疗的慢性丙型肝炎患者血清中总三碘甲状腺原氨酸、总甲状腺素、游离三碘甲状腺原氨酸、游离甲状腺素和促甲状腺激素(thyroid stimulating hormone,TSH),并收集患者临床资料,分析甲状腺疾病发生情况及其对病情的影响。结果 133例慢性丙型肝炎中16例出现甲状腺疾病,其中3例甲状腺功能亢进,7例甲状腺功能减低,6例亚临床甲状腺功能减低。2例低T3综合征。甲状腺功能异常组与甲状腺功能正常组在ALT、AST、TP、HCVRNA定量、肝硬化发生率及合并症方面差异无统计学意义,但甲状腺功能异常组ALB水平明显低于甲状腺功能正常组。结论 12.0%慢性丙型肝炎患者存在甲状腺功能的异常,TSH的检测较其他甲状腺激素的测定更灵敏;甲状腺功能异常对慢性丙型肝炎患者的肝功能、病毒复制无明显影响。Objective To investigate the occurrence of thyroid diseases and its relationship with liver function, viral replication, the development of liver cirrhosis and complications in patients of chronic hepatitis C （CHC）. Methods Serum total triiodothyrone, total thyroixine, free triiodothyrone, free thyroixine and thyroid stimulating hormone （TSH） of 133 HCV patients were detected and clinical data were investigated to analyze the impact of the development of thyroid diseases on the disease state of CHC. Results Of 133 CHC patients, 16 developed thyroid disorders, 3 with hyperthyroidism, 7 with hypothy-roidism and 6 with subclinical hypothy- roidism. Another 2 patients had been diagnosed with low T3 syndrome. The levels of ALT, AST, TP and HCV RNA, the occurrence of liver cirrhosis and complications were not significantly different between the normal thyroid function group and the thyroid dys- function group, and ALB level of the thyroid dysfunction group was significantly lower than that of the normal thyroid function group. Conclusions Of 133 CHC patients, 12% have thyroid dysfunction, and the detection of serum TSH is more sensitive than that of other thyroid hormones. Thyroid dysfunction has no impact on liver function and HCV replication in the CI-IC patients.
