健康管理对围绝经期综合征患者激素水平的影响

摘　　要：目的探讨健康管理干预方法对围绝经期综合征(PS)患者激素水平的影响。方法采用健康管理的思路与方法,对94名PS患者进行问卷调查、信息采集及风险评估;经Logistic回归分析显示"对PS知识获取不足"、"过度焦虑心理"及"缺乏运动"是PS排名前3位的危险因素。将以上患者分为干预组(n=48)及对照组(n=46),后者单纯给予谷维素10 mg,tid;前者给予健康管理干预方法,包括知识宣教、心理调节(音乐放松法、转移注意力、主动倾诉等)以及运动调节(健身运动、跳舞、太极拳等),并进行为期1年的随访,干预前后检测激素含量。结果干预前两组患者激素含量无统计学差异(p>0.05);干预后两组患者的激素水平均有不同程度的改变,总体表现为雌激素、孕酮及催乳素水平的上升,雄激素及促性腺素含量的下降;除雄激素及催乳素外,其余3种激素的组间含量差异均有统计学意义,均以干预组的改变较对照组明显(p<0.05),且干预组的躯体症状缓解较对照组理想。结论健康管理对PS患者性激素代谢紊乱具有一定的调节作用,是一种有效、廉价、非药物的健康干预措施。Objective To explore the essential effect of health management（HM） to intervene the sex hormone of the patients with perimenopause syndrome（PS）.Methods A standardized procedures of HM was performed and a questionnaire survey was undertaken for the 94 subjects.Logistic analysis showed that the risk factors for the subjects were： lack of PS knowledge,anxious and lack of sport.The 94 patients were divided into the observation group（n=48） and the control group（n=46）,the latter ofwhich were only given oryzanol 10mg,tid,whereas the observation ones were given the HM intervention.A one-year follow up was carried out.Results Before intervention,there was no statistical difference between the observation and control group about the sex hormone（p0.05）.After intervention,the hormone level of both groups has changes in different degrees;collectively present the increase of estrogen,progesterone and prolactin level and the decrease of the androgen and gonadotropin level.Beside androgen and gonadotropin,the other three hormones have statistical significant difference of content between two groups;all are more significant in intervention group than control group（p0.05）.Otherwise,the remission of body symptoms is rather ideal in intervention group than control group.Conclusion HM has definite adjust effect on the disturbance of sex hormone of patients with PS,which was a low-cost and feasible way to treat the PS.
