V形皮瓣龟头成形术在尿道下裂治疗中的应用

摘　　要：目的：寻找一种尿道下裂治疗中防止龟头成形术后尿道外口狭窄的方法。方法：１１ 例阴茎阴囊型或阴囊型先天性尿道下裂患儿，在行阴囊纵隔岛状皮瓣尿道成形术中应用了“ Ｖ”形皮瓣龟头成形术，术后对排尿情况和尿道外口大小进行了随访和测量。结果：在１ ～３５ 年的随访中，除１例在原尿道外口处有轻度狭窄外，其余均未见有龟头末端尿道外口狭窄，而且尿流通畅。结论：“ Ｖ”形皮瓣龟头成形术是一种防止尿道重建后尿道外口狭窄的有效方法。OBJECTIVE:To find a better procedure to prevent the meatal stenosis in glanuloplasty for the patients with hypospadias. METHODS:Eleven patients with penosacrotal or sacrotal type hypospadias were operated by a new procedure——V-shape glans flap glanuloplasty in one-stage sacrotal midline island flap urethroplasty.Postoperalive urination and measurement of the meatus were observed.RESULTS:In 1-1.5 years follow-up all cases demonstrated no meatal stenosis with good urinary flow except one case urethral restricture in the junction part between neourethra and the original outlet.The meatus diameter was 0.5 cm averagely.CONLUSION:V-shape glans flap glanuloplasty is a good technique valuable to be used in complete reconstruction of the urethra with meatus at the tip of the penis.
