老年心力衰竭患者B型利钠肽水平与心功能的相关性

摘　　要：目的:探讨老年心力衰竭患者血中B型利钠肽(BNP)水平与心功能分级,左室结构、功能的相关性。方法:入选72例老年心力衰竭患者,并以37例心功能正常的健康老年人作为对照组。以荧光免疫检测法测定各组血BNP水平,应用多普勒超声心动图测定左心室舒张末内径(LVEDD)、左心室射血分数(LVEF),分析血BNP水平与心功能的相关性。结果:老年心力衰竭患者BNP水平高于对照组,且BNP水平随着心功能分级增高而升高,不同心功能级别BNP水平差异有统计学意义(P<0.05)。血BNP水平与LVEF呈负相关(rs=-0.51,rs′=-0.52,P<0.01),与LVEDD呈正相关(rs=0.36,rs′=0.35,P<0.01)。结论:血BNP可作为判定老年人心力衰竭及其严重程度的指标。Objective： To analyze the relationship between heart function and Prtype natriuretic peptide （BNP） in elderly patients with chronic heart failure （CHF）. Methods： A total of 72 elderly patients with chronic heart failure and 37 healthy elderly were selected. 72 elderly patients with CHF were divided into Ⅱ ,Ⅲ , and IV NYHA functional class. Plasma BNP was measured. Left ventricular ejection fraction （LVEF） and end diastolic diameter （LVEDD） were examined with echocardiography. Results：Plasma BNP concentration increased significantly in elderly patients with CHF compared with that in healthy elderly （P〈0. 05）, and increased progressively in proportion to the degree of NYHA （P〈0.05） ; BNP level was negatively correlated with LVEF （rs = - 0.51 , rs＇ = - 0.52, P〈0.01）, and positively with LVEDD （rs = 0. 36, rs＇ = 0. 35, P〈0. 01）. Conclusion： BNP might be used as an objective indicator for the severity of in left ventricular dysfunction elderly patients with CHF.
