无创正压通气治疗慢性阻塞性肺病合并Ⅱ型呼吸衰竭临床应用与护理

摘　　要：目的观察无创正压通气治疗慢性阻塞性肺病(COPD)合并Ⅱ型呼吸衰竭应用效果并总结护理经验。方法 72例患者采用无创呼吸机通气。观察治疗前、后pH值、心率(HR)、呼吸频率(RR)、血气分析等指标的变化情况。结果治疗前后pH值变化不明显,差异无统计学意义(P>0.05)。治疗后PaO2较治疗前升高,PaCO2、HR、RR较治疗前均降低,差异均有统计学意义(P<0.05)。结论无创正压通气治疗COPD合并Ⅱ型呼吸衰竭效果显著,合理的护理措施能有效防止并发症,值得临床推广应用。Objective To observe the effect of noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure,and to summarize the nursing experience.Methods 72 patients were treated by noninvasive respiratory machine.PH value,heart rate（HR）,respiratory rate（RR）,blood gas analysis and other indicators were observed before and after treatment.Results The PH value had no noticeable change before and after treatment,so there was no statistical significance（P0.05）.The oxygen partial pressure（PaO2） increased after treatment compared with before treatment,but the PaCO2,HR and RR decreased compared with before treatment,there were significant differences before and after treatment（P0.05）.Conclusion Noninvasive positive pressure ventilation in the treatment of patients with COPD with type Ⅱ respiratory failure is effective.And reasonable therapeutic measures can effectively prevent complications.So it is worth clinical popularizing and application.
