感觉神经根切断与微血管减压术治疗原发性三叉神经痛的疗效比较

摘　　要：目的分析原发性三叉神经痛感觉根切断(Dandy术)与微血管减压(MVD)两种术式的疗效。方法原发性三叉神经痛患者87例,Dandy术35例,MVD术52例,比较两种术式的疗效、并发症和复发情况。结果本组无重度残疾和手术死亡。与MVD术相比,Dandy术缓解率更高,而失败率更低,术后的复发率也更低。Dandy术的面部感觉异常、角膜感觉减退的发生率高于MVD术。结论 Dandy术干扰小、复发率低、疗效可靠,可放宽其应用指征。Objective To compare the outcomes of treating primary trigeminal neuralgia by Dandy and microvascular decompression（MVD） surgeries.Methods Of 87 cases with primary trigeminal neuralgia,52 cases（group M） received MVD surgery and 35 cases（group D） were operated with Dandy operation.The efficacy,complications and recurrence were compared between two groups.Results There was no case with severe disability or operative death.Compared to group M,the patients in group D had higher remision rate,lower failure rate and less recurrence rate.More cases with facial paraesthesia and corneal hypoesthesia were found in group D than those in group M.Conclusion Dandy operation is an effetive surgical method in treating primary trigeminal neuralgia with little surgical interference and low relapse rate.The indications of Dandy operation could be wildened.
