152例肝血管瘤的临床治疗分析

摘　　要：目的:比较手术切除、肝动脉栓塞术、经皮肝穿刺微波治疗3种方式治疗肝血管瘤的疗效,探讨肝血管瘤合理的治疗方法。方法:回顾性分析2006年1月—2009年6月在山东大学齐鲁医院住院治疗的肝血管瘤病例152例,其中91例行手术切除,31例行肝动脉栓塞术,23例行经皮肝穿刺微波治疗,7例进行分期治疗。通过病例对照分析及随访结果综合评价肝血管瘤的治疗效果。结果:肝动脉栓塞术及经皮肝穿刺微波治疗与手术切除在减少手术时间、缩短术后住院天数、节省总住院费用、降低术后并发症方面有明显优势,但肝血管瘤的复发率比手术切除高。结论:肝血管瘤的治疗需依照个体化原则,根据病人全身状况,血管瘤的数目、大小和位置选择合理的治疗方法。Objective： To compare the curative effect of three treatment methods in the treatment of hepatic hemangioma-surgical resection,therapevtic arterial embolization and percutaneous transhepatic microwave therapy,and investigate reasonable treatment.Methods： One hundred and fifty two cases of hepatic hemangioma from January 2006 to June 2009 in Qilu Hospital of Shandong University were statistically analyzed,of the 152 cases,91 cases had surgical resection,31 cases had therapevtic arterial embolization,23 cases had percutaneous transhepatic microwave therapy,and the other 7 cases had two-stage treatment.Hepatic hemangioma treatment effects were comprehensively evaluated through case-control analysis and follow-up results.Results： The applied ascendancy of therapevtic arterial embolization and percutaneous transhepatic microwave therapy in reducing duration of surgery,shorting hospitalization after operation,economizing overall cost for hospitalization and reducing complications postoperative were obviously better than surgical resection.But the follow-up results in the recurrence rate of hepatic hemangioma was higher than the surgery group.Conclusion： The treatment of hepatic hemangioma should follow the individualized principle,and we should choose appropriate treatment according to systemic status of patients and the number,size and location of hemangiomas.
