摘  要:目的:分析新生儿听力筛查中瞬态耳声发射的应用、意义以及其局限性。方法:运用瞬态耳声发射(TEOAE)对出生72~120小时的新生儿进行听力筛查,未通过初筛者42天内进行复筛,复筛未通过者3~6个月内用脑干诱发电位(ABR)进行听力损失的诊断。对通过TEOAE筛查的具有高危因素的新生儿随访半年,对疑有先天性听力损失者行ABR检查,作出听力损失的诊断。结果:8024例新生儿中有6919例通过初筛,通过率为88.23%,1034例通过复筛,两步筛查法的总通过率为99.12%,71例新生儿接受ABR检查,有13例诊断有不同程度的听力损失,对TEOAE通过但具有高危因素,疑有先天性听力损失的新生儿共58例,接受ABR检查,有3例诊断有不同程度的听力损失,由本资料显示:TEOAE的假阳性率为0.72%,假阴性率为0.37‰。结论:TEOAE是适用于新生儿听力筛查一种快速、简便、无创的方法。但具有一定的局限性,分析新生儿听力筛查中出现的假阳性和假阴性因素,提高听力筛查的质控水平。Objective:To investigate the probabilities of newborn hearing screening in TEOAE of application,as well as its limitations.Methods: Used TEOAE to screen of neonatal hearing test who born after 72~120 hours,fails the preliminary screening were secondary screened within 42 days,who screen failed would be use ABR to diagnosis hearing loss.On through TEOAE screening,those have risk factors for neonatal follow—up for half a year,on suspected congenital hearing loss who rows ABR checks,making the diagnosis of hearing loss.Results:Section of the 8024 neonates in 6919 cases through the preliminary screening,the pass rate for 88.23%,1034 cases through secondary screen,two step screening method always pass rates for 99.12%,71 neonates receiving ABR check,13 cases of diagnosis have varying degrees of hearing loss.TEOAE through but with risk factors,suspected congenital hearing loss in newborns total 58 cases,accept ABR check,3 cases of diagnosis have varying degrees of hearing loss,this information is displayed:TEOAE of false—positive rate 0.72%,false negative rate of 0.37‰。Conclusions: TEOAE is applicable to the newborn hearing screening in a fast,simple,non—invasive methods.But with certain limitations,analysis of newborn hearing screening in false positive and false negative factors that increase the level of quality control of hearing screening.
