舒适护理在高龄产妇中应用的效果

摘　　要：目的探讨舒适护理对高龄产妇分娩期心理、生理的影响。方法将100例高龄产妇随机分为实验组（50例）与对照组（50例），对照组给予产科常规护理；实验组在产科常规护理的基础上给予分娩全程舒适护理。实施舒适护理前后采用焦虑自评量表（SAS）对两组产妇进行评定，比较两组产妇心理状态及自然顺产率、产后出血率、产后乳汁不足的发生率。结果舒适护理前两组产妇SAS评分比较差异无统计学意义（P〉0．05）；舒适护理后实验组SAS评分为（34．25±8．61）分，对照组SAS评分为（38．36±5．85）分，差异有统计学意义（t=2．79，P〈0．01）；实验组产妇自然顺产率高于对照组（78．00％比56．00％，X2=5．47，P〈0．05），疼痛耐受性高于对照组（96．00％比68．00％，x2=13．28，P〈0．01），产后出血率、乳汁不足发生率低于对照组（P〈0．01）。结论舒适护理能缓解高龄产妇的焦虑情绪，减轻分娩期的疼痛，降低产后出血、乳汁不足并发症发生率，提高顺产率，提高产妇心理、生理舒适度。Objective To investigate the psychological and physiological effect of comfort nursing to elder puerperants. Methods 100 elder puerperants were randomly divided into experimental group （n = 50） and the control group （ n = 50 ）, control group received routine obstetric care; the experimental group received comfort nursing in the whole course in addition to routine obstetric care. The anxiety scale （SAS） was used before and after nursing to compare their mental status and the incidence rate of eutocia, postpartum hemorrhage and postpartum shortage of milk secretion. Results The score of SAS has no significant difference before nursing（P 〉 0. 05 ）, after comfort nursing the SAS scores of experimental group were significantly lower [ （34.25 ± 8.61 ）vs （ 38.36 ±5.85 ） ,t = 2. 79,P 〈 0. 01 ] ; The rate of eutocia is higher than the control group （ 78.00% vs 56.00%, X2 =5.47,P 〈0. 05）, pain tolerance is higher than the control group（96.00% vs 68.00% ,X2 = 13.28,P 〈 0. 01 ） ; and incidence rate of postpartum hemorrhage and postpartum shortage of milk secretion were lower than the control group （ P 〈 0. 01 ）. Conclusions Comfort nursing can ease the anxiety of elder puerperants, alleviate intrapartum pain, reduce postpartum hemorrhage, insufficient milk complication rate, improve eutocia rate and make elder puerperants comfortable psychologically and physiologically.
