亚低温联合胸腺五肽对重型脑损伤患者T细胞亚群及预后的影响

摘　　要：[目的]研究亚低温联合胸腺五肽对急性重型颅脑损伤患者T细胞亚群及预后的影响。[方法]重型颅脑损伤患者共42例,随机分为亚低温联合胸腺五肽组(实验组)和亚低温治疗组(对照组),实验组患者22例,于伤后24h内行亚低温治疗同时给予胸腺五肽静脉滴注及常规治疗。对照组患者20例,除未应用胸腺五肽外其余治疗相同。两组分别于伤后d1、d3、d7取外周静脉血采用流式细胞术检测T淋巴细胞亚群;观察两组病人院内感染情况及预后。[结果]两组T细胞亚群之间比较,伤后d1差异无统计学意义,伤后d3、d7除了CD8+细胞外差异均有统计学意义;院内感染发生率两组间差异有统计学意义;预后两组间差异有统计学意义。[结论]亚低温联合胸腺五肽组治疗重型颅脑损伤可以改善T细胞亚群受损情况,降低院内感染发生率,提高患者的预后。[Objective]To study the effect of mild hypothermia combined with TP-5 on T lymphocyte subsets and prognosis of patients with severe brain injury.[Methods]All 42 patients with severe brain injury were randomly divided into the mild hypothermia combined with TP-5 group（22 cases）and hypothermic group.Giving TP-5 with intravenous drop infusion,experimental group was induced mild hypothermia within 24 hours of injury.The therapeutic regimen of control group was the same with experimental group except for TP-5.T lymphocyte subsets of venous blood were detected in two groups on 1st,3rd,7th days after injury by flow cytometry.Hospital onset of infection was observed between the two groups.According to GOS,the prognosis of the patients was evaluated.[Results]In comparison with control group,T lymphocyte subsets didn＇t have difference on 1st day after injury,but there were differences on 3rd and 7th days after injury by flow cytometry except for CD8＋.There were no differences between the two group in the prognosis.[Conclusion]Mild hypothermia combined with TP-5 treatment could improve T lymphocyte subsets and decrease hospital onset of infection rate,and improve the prognosis of patients.
