军地病人病例分型与医疗费用的比较

摘　　要：目的对某军队医院6年来军队和地方住院患者病案质量医疗费用进行对比分析,对医院的医疗费用和医疗质量管理提出建议。方法用SPSS12.0统计软件对2001年-2006年出院病人123 618份病案首页进行描述性分析,用多元线性回归找出影响人均医疗费用的相关因素。结果军队病人和地方病人在人均医疗费用上有明显的差异。病人的病例分型、身份、平均住院日的长短等,都是影响人均医疗费用的关键因素,病例分型与医疗费用相关性体现了医疗合理性。结论医院在军队病人的管理上,要进一步落实分类保障,科学计价;对地方病人要加强管理,引导合理医疗。Objective Having a comparative analysis on medical record quality and medical expenses between military and local hospitalization patient of a military hospital,put forward suggestion about medical expenses and medical quality management.Methods 123,618 copies of front page of medical record have been descriptively analyzed from 2001 to 2006 by SPSS12.0 statistic software,finding out the related factors affecting medical expenses per capita by using multiple linear regressions.Results There are obvious differences of medical expenses per capita between patients in army and in local.The key factors affect medical expenses are case classification,identity and time of the average hospitalization day.The relativity between case classification and medical expense embodies the medical rationality.Conclusions The hospital needs to further implement classification insurance and scientific pricing in management of military patients;it should strengthen management about local patients for leading reasonable medical.
