同期实施不停跳冠状动脉旁路搭桥及肺叶切除术患者的围手术期护理

摘　　要：目的总结同期实施不停跳冠状动脉旁路搭桥及肺叶切除术患者围手术期的护理经验。方法回顾性分析2000年3月-2009年11月本院收治的10例肺部肿瘤合并冠心病同期实施不停跳冠状动脉旁路搭桥及肺叶切除术患者的临床资料,并总结护理要点。结果 10例患者术程顺利,手术时间4.2-7.9h,平均(5.9±1.5)h;住院天数12-34d,平均(20.0±9.2)d;术后发生心房颤动3例,低心排综合症1例,无1例死亡。所有患者心肺功能恢复良好,均痊愈出院。结论术前做好心理护理、功能锻炼和呼吸道准备,术后严密监测病情的变化,及时发现并发症并协助医生处理,是同期实施不停跳冠状动脉旁路搭桥及肺叶切除术取得成功的关键。Objective To summarize the nursing experience for patients who received lung resection synchronous with off-pump coronary artery bypass grafting（OPCABG）.Methods Clinical data of ten patients who received lung resection synchronous OPCABG from Mar 2000 to Nov 2009 were retrospectively analyzed.The nursing experiences were summarized.Results All cases recovered successfully.Among them 8 patients with lung cancer received lobe and lymph nodes resection.The other 2 patients with benign disease only had local lung excision.The operation time was 4.2 ～ 7.9 h.The average hospital day after operation was（20.0±9.2） days.Postoperative complications included atrial fibrillation（3 cases） and heart failure（1 case）.No patients died.Cardiorespiratory function was recovered well in all patients.Conclusions Pre-operative psychological nursing,function exercise and airway preparation were very important.Carefully nursing and preventing the complication were one of key points for successful lung resection synchronous with OPCABG.
