老年糖尿病患者红细胞胰岛素酶活性研究

摘　　要：目的　探讨年龄与病程对Ⅱ型糖尿病患者的胰岛素降解速度的影响。方法　对老年Ⅱ型糖尿病患者４９ 例、非老年Ⅱ型糖尿病患者７１ 例以及正常对照６０ 例，检测了红细胞胰岛素酶活性（ＥＩＡ）、空腹血浆胰岛素（ＦＩＮＳ）、空腹血糖（ＦＰＧ）和糖基化血红蛋白（ＨｂＡ１ｃ） ，并以ＦＩＮＳ和ＦＰＧ乘积的倒数作为胰岛素敏感性指数（ＩＳＩ） 。结果　老年及非老年Ⅱ型糖尿病患者的ＥＩＡ 显著高于正常对照组，其中，老年Ⅱ型糖尿病患者的ＥＩＡ 和ＦＰＧ 高于非老年患者，而ＩＳＩ显著低于非老年患者。相关分析表明，２ 组糖尿病患者的ＥＩＡ 与病程和ＦＩＮＳ呈显著负相关。结论　老年Ⅱ型糖尿病患者的红细胞胰岛素降解速度升高，而ＩＳＩ下降，提示随着年龄增大和病程延长，Ⅱ型糖尿病患者的胰岛素抵抗逐渐加重。Objective\ To study the effect of age and duration on the rate of insulin degradation of erythrocytes in patients with type 2 diabetes mellitus(DM).Methods\ Insulinase activity of erythrocytes(EIA),fasting plasma insulin(FINS),fasting plasma glucose(FPG)and glycosylated hemoglobin(HbA 1c)were determined in 49 aged patients with type 2 DM,71 non-aged patients with type 2 DM and 60 healthy controls.Moreover,the reverse of FINS and FPG products (1/FINS×FPG)was used as the insulin sensitivity index(ISI).Results\ EIA was higher and ISI was lower in both of aged and non-aged patients than those in the controls.In aged patients,EIA and FPG were increased and ISI was decreased,as compared with non-aged patients.Moreover,EIA was inversely correlated with duration of diabetes,FINS and age in the two groups of patients with type 2 DM.Conclusion\ Insulin resistance will turn serious,accompanying the older age and the longer duration of diabetes.
