心脏不停跳下保留二尖瓣结构的二尖瓣替换术的临床观察

摘　　要：目的 :探讨在心脏不停跳下保留二尖瓣结构的二尖瓣替换术临床应用价值。方法 :对 1994年 7月至 1997年 12月在浅低温体外循环心脏不停跳下对 15例患者行保留二尖瓣结构的二尖瓣替换术并与同期不保留二尖瓣结构的 12例比较。结果 :术后病情较稳定 ,心功能恢复较快 ,全部存活出院。两组相比 ,保留组 SVI、EF改善明显 ,左房、左室缩小显著。结论 :在二尖瓣替换术中保留二尖瓣结构有维持左心室舒缩功能作用 ;在浅低温体外循环心脏不停跳下手术不存在心脏停跳后再灌注问题 ,起到减轻心脏损伤的作用 。Objective:To prope into the clinical value of mitral valve replacement by reserving the structures of valve with heart beating Methods:From July,1994 to December,1997,we had 15 cases of valve replacement with valve structure reserved and 12 cases without in low hypothermia and extracorporeal circulation with heart beating Results:All these patients' conditions were stable and rescoverel soon after operations SVI & EF of the reservation group improved more obviously than the non reservation group Conclusion:It's useful to keep the function of left ventricle of heart during mitral valve replacement,and there was no problem in re perfusion injury It could reduce the cardiac injury and help to detect and correct the functional disorder of prosthetic valve
