人工全髋关节置换术肢体长度调定方法的前瞻性研究

摘　　要：探讨人工全髋关节置换术预防双下肢不等长的方法。综合采用术前X线片模板测量股骨头运动中心指导选择假体颈长;术中根据股骨头运动中心是否与大粗隆尖端平齐、侧卧位测量双髌骨下缘是否在同一平面、牵拉患肢根据软组织张力调整患肢长度等方法来确定术中股骨颈假体的长度。本组44例患者分为2组:A组术前肢体等长(21例);B组术前肢体不等长(23例)。对44例患者全髋关节置换进行了统计分析,术前双下肢长度差异的均值为:A组-0.1±0.2cm,B组-1.2±0.3cm;术后双下肢长度差异的均值为:A组0.1±0.1cm,B组-0.1±0.2cm。A组患者基本获得了肢体等长,B组长度差异在0.3cm以内,结果令人满意。该方法是一种人工全髋关节置换手术肢体长度调整较可靠的方法。Discussed the Preventing method of different length of lower limbs alter Total Hip Arthroplasty（THA）. Chose the length of artificial neck under the guide of measuring the motor center of femoral head by X--ray template before the operation. Three methods were used to prevent different length of lower limbs ： measure double large promontories on similar line, measure the lower borders of both patellas in the same line, pull--draw test to judge the length of both thighs,and then,made the decision of the artificial neck length according to the tension of soft tissue. 44 patients from March,2006 to March,2007 were divided into two groups,A group with similar length of both limbs, B group with different length. The difference of double thighs was as follows, A group before operation：-0.1±0.2cm;B group：-1.2± 0.3cm. After operation A group：0.1±0.1cm; B group： - 0.1±0.2cm. The difference of lengths was among 0.3cm in all cases. The results were satisfied. They were ideal method to Prevent different lengths of lower limbs after THA and had applicable value.
