摘  要:目的应用超声二维应变成像技术(2DS)定量评价肺动脉高压患者右室收缩功能。方法连续入选肺动脉高压患者41例(PH组),正常人40例(对照组),应用2DS测定右室游离壁基底段、中间段及心尖段纵向应变(ε)、应变率(SRs),并与传统超声及右心导管结果对比。结果 (1)PH组右室游离壁各节段ε、SRs均较对照组减低,差异均有统计学意义(P<0.05)。PH组右室游离壁的中间段及心尖段ε、SRs测值明显低于基底段。(2)右室游离壁心尖段、中间段SRs与右室面积及右室内径测值相关性良好。右室心尖段ε与右室大小及右室整体收缩功能均有良好相关性。(3)PH组右室游离壁心尖段ε、SRs与右心导管测量肺动脉压力、肺血管阻力及每搏量相关性良好。结论二维应变技术可更准确、客观地评价右室长轴纵向收缩功能。Objective To evaluate RV longitudinal function in pulmonary hypertension (PH) patients by Two-dimensional strain (2DS). Methods 41 patients with PH were prospectively studied along with 40 control patients. RV 2DS and right heart catheterization were performed,peak systolic longitudinal strain(e) and strain rate(SRs) obtained from basal to apical RV free wall were calculated. RV Deformation indices were compared with values of conventional echocardiography and right heart catheterization with the use of linear regression. Results (1) ε, SRs of all RV free wall segments in PH group were significantly lower than those in the control group (P 〈0.05) ; ε, SRs of the middle and apical segment in PH group were significantly lower than basal segment (P 〈0.05). (2) SRs of RV apical and middle segment in PH group correlated well with RV size and diameter. Also, ε of RV apical segment correlated well with Conventional echoeardiography measurement of RV size and systolic function. (3)ε, SRs of the apical segment in PH group correlated well with right heart catheterization measurement of pulmonary artery pressure, pulmonary vascular resistance and stroke volume. Conclusions The right ventricular longitudinal systolic function can be evaluated accuratelv and obiectivelv bv Two-dimensional strain Echocardiography.
