超声引导下^(125)I放射性粒子近距离治疗前列腺癌

摘　　要：目的探讨超声引导下经会阴穿刺植入^(125)I放射性粒子治疗前列腺癌的操作技术及临床应用价值。方法对26例确诊前列腺癌患者实施经直肠超声引导下经会阴^(125)I粒子植入术。术中对前列腺进行超声扫描,将图像传送到计算机制定种植治疗计划并在种植前计算放射线对前列腺体积的有效覆盖。其中23例行单纯粒子治疗,3例术后行外放疗。结果 23例单纯粒子植入治疗前和治疗3个月后血清前列腺特异抗原PSA水平明显降低,分别为(17.60±22.43)ng/mlvs(0.13±0.17)ng/ml(P%0.01),3例术后配合外放疗治疗前和治疗后3个月血清PSA分别为(22.70±36.67)ng/ml vs(0.22±0.30)ng/ml(P>0.05)。结论超声引导经会阴^(125)I放射性粒子植入治疗前列腺癌具有安全、微创、并发症发生率低等优点。Objective To evaluate the clinical application of permanent ^125I brachytherapy in patients with prostate cancer. Methods 26 prostate cancer patients underwent 3-dimensional^125I seeds implantation brachytherapy with transrectal ultrasound-guide. 3 patients were treated by external radiotherapy after, brachytherapy. Results 23 patients with single ^125I brachytherapy mean blood serum PSA were （17.60 ± 22.43）ng/ml before treatment and （0.13±0.17） ng/ml three months later after treatment with significant difference（P〈0.05）. 3 patients were combined with external radiotherapy. Their mean blood serum PSA were （22.70±36.67） ng/ml before treatment and （0.22 ±0.30） ng/ml three months later after treatment with nonsignificant difference （P 〉 0. 05 ）. Conclusions Brachytherapy with ^125I seeds implantation guided by transrectal ultrasound is a safe,minimally invasive and effective treatment with low morbidity for prostate cancer patients.
