平板运动试验强阳性患者的临床特征

摘　　要：目的　研究平板运动试验强阳性患者的冠状动脉病变及心血管事件、预后和治疗状况。方法　１９９５ 年初至１９９８ 年１０ 月，２１５ 例因胸痛拟诊冠心病患者在我院作冠状动脉造影术（ＣＡＧ） ，术前１ 周内均实施平板运动试验（ＴＥＴ） 。其中男性１７７ 例，女性３８ 例，年龄（５９ ±１０） 岁，ＴＥＴ 表现强阳性５８ 例，阳性５４ 例，阴性１０３ 例，作者对三组患者做了冠状动脉病变分析、２ 个月内心血管事件及猝死发生的随访。结果　（１） 强阳性患者几均为内科治疗无效病例。（２） 几乎均有严重多支复杂病变。（３） ２ 个月随访期内，强阳性者６ 例发生严重心血管事件，５ 例死亡。结论　运动试验强阳性预示冠心病严重，临床表现和治疗上有其特殊性，应予高度重视。Objective To study the severity of coronary artery disease, the incidence of cardiac events, prognosis and therapeutic choice of patients with strongly positive results of treadmill exercise testing (TET). Method From 1995 to October of 1998,215 subjects suspected of coronary artery disease were hospitalized and examined with TET one week before coronary arteriography (CAG), among them, 177 were males, 38 were females, with a mean age of 58.6±9.8 years old. 58 cases showed strongly positive results, 54 were positive and 103 negative, then, within a period of 2 month follow up coronary lesions, and sudden death were analyzed among the three groups.Result (1) Almost all patients with strongly positive TET had no response to the treatment with antianginal drugs. (2) During the 2 month follow up period, 6 had serious cardiovascular events, and 5 died. (3) Almost all of them showed grave and complex multivascular disease. Conclusion The strongly positive TET result always indicates severe coronary heart disease and is unique in clinical manifestations and treatment.(Shanghai Med J, 1999,22∶687 688)
