腰椎椎弓崩裂并滑脱的CT诊断

摘　　要：目的：提高对腰椎椎弓崩裂并滑脱的ＣＴ征象的认识。方法：利用ＧＥ公司的ＣＴＭＡＸ６４０扫描仪对２７例椎弓崩裂病人进行扫描，将其ＣＴ征象分为崩裂部表现，滑脱表现及其他表现三部分进行分析。结果：崩裂部主要为不规则破碎状断裂，常局部粗大并有碎骨片；滑脱共２０例，主要征象有椎管矢状径及硬膜囊矢状径增大，椎间盘假性膨出，夹心征，叠椎征以及椎间孔的变形；其他征象有小关节不对称，椎弓和椎板发育畸形以及椎板和棘突的骨密度改变等。结论：ＣＴ扫描不仅能明确诊断而且可以显示许多细节，采用椎间孔扫描法有助于诊断。Objective:To improve the knowledge of CT features of spondylolysis and spondylolisthesis of the lumbar spine.Methods:The CT appearances of spondylolysis and spondylolisthesis of the lumbar spine were analyzed in 27 cases.CT fiadings included:defect of the pars interarticular,anterior displacement of the involved vertebra,and other features.Results:The sign of spondylolysis was irregular break zone at the pars interarticular and fragmentation of the lamina adjascent to the defect.Spondylolisthesis in 20 cases manifested as the anterior pasterior diamenter elongated of the canal and/or dural sac,pseudobulging disk,wich sign,overlapped vertebrae and deformity of neural foramen.Other features included asymmetrical face joints,anomalous laminae and altered density of laminae.Conclusion:CT can show a lot of details of spondylolysis and spondylolistesis of lumbar spine,CT scanning of.neural foraman is helpful for diagnosis.
