三种男性绝育术后抗精子抗体的比较

摘　　要：本文对直视钳穿输精管结扎术、输精管注射粘堵术和可复性输精管注射栓堵术手术前后血清抗精子抗体的发生情况进行了连续观察并加以比较,客观评价了3种绝育术后免疫反应情况。采用间接免疫珠试验和浅盘凝集试验方法检测术前、术后3、6、12、24个月时血清中的抗精子抗体。结果:3种绝育方法术后3个月凝集抗体阳性率和滴度均明显高于术前,直视钳穿、粘堵、栓堵凝集抗体阳性率最高时分别为46.2％、42.2％和50.0％;用免疫珠试验检测3种绝育术后3个月的血清,均已有抗体产生,直视钳穿、粘堵、栓堵免疫珠抗体阳性率最高时分别为26.9％、38.5％和37.8％。经相关分析显示,用免疫珠试验和浅盘凝集试验检测3种术式的抗体阳性率间有良好的相关性。结果表明,输精管绝育术后可产生抗精子抗体,可复性输精管注射栓堵术后抗体阳性率稍高,其中术后12个月栓堵凝集抗体阳性率明显高于直视钳穿输精管结扎术,术后24个月栓堵免疫珠抗体阳性率明显高于粘堵。各术式免疫珠抗体以IgG为主。The incidences of serum antisperm antibodies from the men underwent no - scalpel vasectomy(N), chemical vas occlusion (C) and medical - grade polyurethane vas occlusion (MPU) were monitored and compared, in order to provide objective assessment on immunoreaction post male sterlization. The serum samples from men underwent N, C and MPU were analyzed for antisperm antibodies by using immunobead test(IBT)and tray agglutination test(TAT)at 3、6、12and 24 months after operation, respectively. The results showed that the incidences of the agglutinating antibodies and their titers at 3 months after operation were significantly increased. The highest positive rates (TAT) of NX and MPU were 46.2%、42.2% and 50.0%, respectively. Serum immunobead antibodies were detected at 3 months after operation. The highest positive rates (IBT) of N、C and MPU were 26.9%、38.5% and 37.8%,respectively. There was strong correlation between the positive rates of immunobead antibodies and those of agglutinating antibodies. The present date indicated that there were serum antisperm antibodies from the men who underwent sterilization. The positive rates (both TAT and IBT)of MPU were slightly higher than those of N and C. However, there were significant differences between MPU and N at 12 months after operation (TAT), and between MPU and C at 24 months after operation(IBT),only. IgG was main antisperm antibody in IBT for the three groups of men after operation when classifying it .
