摘 要:目的 研究有机磷农药中毒与心脏损害的关系,更好地改善临床治疗。方法 对92 例有机磷农药中毒病例进行系统分析。结果 发现患者心电图异常发生率高,改变呈多样,且与中毒程度相关,存在剂量反应关系;心电图的异常与肺水肿的发生无关;心电图异常者中心肌酶谱改变的比例较高(OR= 5 .79) 。结论 有机磷农药中毒患者的心电图异常可能是原发直接性的,是有机磷农药对心肌细胞直接损伤的结果。Objective To study the relationship between organophosphorus insecticides (OPIs) intoxication and cardiovascular damage in order to improve its clinical treatment. Methods 92 OPIs intoxication patients with ECG records were systematically studied. Results The abnormal rate of ECG was found to be 55.4% and closely correlated with severity of poisoning ( P <0.01). The major types of ECG changes included ①arrhythmia, ②disturbances of conduction, ③repolarization abnormality (ST T changes). High percentage of abnormality of myocardial enzymes was also observed, suggesting myocardial lesion. Pulmonary moist rate was chosen as an indicator for pulmonary edema, and was found irrelevant to the occurrence of ECG disorders. It might demonstrate that cardiac toxic effect would not be secondary to respiratory failure. Conclusion These indicated that OPIs might have direct/primary toxic effect to the heart, causing an important complication in the intoxicated patients and even fatal outcome.
