关节镜下行踝关节融合术治疗晚期顽固性踝关节疼痛的临床疗效

摘　　要：目的评估关节镜下行踝关节融合术治疗晚期顽固性踝关节疼痛的疗效。方法 2005年10月～2008年1月笔者所在医院收治了20例晚期顽固性踝关节疼痛患者,均采用关节镜行踝关节融合术,镜下清除踝关节内增生充血滑膜,磨削胫距关节面已磨损的关节软骨及其硬化骨,摘除出游离体,并以两枚松质骨无头螺钉固定踝关节功能位。结果术后平均随访26周(20～32周),20例患者术前Mazur评分平均49.5分,术后平均90.7分(P<0.05),三维步态分析术前术后比较,术后患者完全融合时间为14周(12～16周)。融合率为95%。2例未达到骨性融合,经治疗,术后18周时达到骨性融合。结论关节镜下行踝关节融合术是治疗晚期踝关节顽固性疼痛行之有效的方法 。Objective To value arthroscopically assisted ankle joint arthrodesis treatment of intractable ankle pain effect. Methods From October 2005 to January 2008, in our hospital 20 patients with advanced refractory patients with ankle pain. Adopts arthroscopy line ankle arthrodesis, microscopically cleared within hyperemia synovial hyperplasia of ankle, grinding tibial articular surface has worn away from the articular cartilage and harden bone loose bodies, and excise out with two medals cancellous bone headless screws ankle functian bits. Results Average＇follow - up of 26 weeks （20 -32 w）. Mazur score improved from preoperative 49.5 points to postoperative 90.7 points （ P 〈 0.05 ）, three diamensions gait analysis comparison . The time of fusion was 14 weeks（ 12 - 16 w）. The overall fusion reat was 95%. 2 cases did not reach the osseous fusion, the treatment, postoperative 18 weeks to osseous fusion. Conclusion Arthroscopically assisted ankle joint arthrodesis is an effective and safe method of treatment of end - stage intractable ankle pain.
