腹腔镜刮吸法与电切法解剖冰冻胆囊三角的对比分析

摘　　要：目的对比腹腔镜下刮吸法与电切法解剖冰冻胆囊三角的治疗结果,评价两种手术治疗方案的优劣。方法选取2007年12月～2009年12月笔者所在医院收治的腹腔镜胆囊切除术72例,其中术中刮吸法解剖冰冻胆囊三角的患者32例,电切法解剖冰冻胆囊三角的患者40例。对两组患者的术前合并症、手术时间、术中失血量、住院时间、卧床时间、术后并发症等临床指标进行统计学分析。结果随访6个月～2年,平均1.2年。两组患者的术前合并症和卧床时间差异无统计学意义(P>0.05),而手术时间、术中失血量、住院时间、术后并发症发生率等各项功能评价均显示刮吸法组优于电切法组,两组间差异均有统计学意义(P<0.05)。结论刮吸法解剖冰冻胆囊三角行胆囊切除操作相对简单,出血量小,避免了冰冻胆囊三角容易出现医源性胆管损伤及渗血渗液多等问题,住院时间缩短,术后恢复快,并发症发生低,其可作为胆囊切除术中防止胆道损伤的一种较为理想的有效冷分离办法。Objective To comparatively analyze the clinical effects of curettage and aspiration and electric cutting method apllying to frozen gallbladder triangle treatment results,to improve the understanding of curettage and aspiration.and evaluate the merits and defects of the two methods.Methods From December 2007 to December 2009,among 72 patients with our of laparoscopic cholecystectomy,curettage and aspiration of 32 cases of patients with frozen gallbladder triangle method intraoperatively,electric cutting method of 40 cases of patients with frozen gallbladder Triangle.Two groups of patients with Clinical indicators for example preoperative complications,surgical time,intraoperative blood loss,hospital stay,bed time,postoperative complications were statistically analyzed,etc.Results The average followed-up time was 1.2 years ranged from 6 months to 2 years.Two groups of patients with preoperative complications and bed time was no significant difference（P0.05）,and operation time,blood loss,hospital stay,and postoperative complications of the functional evaluation have shown that curettage and aspiration method was superior to electric cutting method group,there were significant differences between the two groups（P0.05）.Conclusion Curettage and aspiration is an effective and satisfactory way of cold separation method to anatomy frozen callbladder triangle to prevent bile duct injury,cholecystectomy operation relatively simple,small amount of bleeding,hospital stay shortly,quicker recovery after operation,less rate of complications and et al.It can avoid iatrogenic bile duct injury to be prone inclined to frozen gallbladder triangle and more bleeding exudate in cholecystectomy.
