Carpentier环在Ebstein畸形治疗中的应用

摘　　要：介绍用Ｃａｒｐｅｎｔｉｅｒ环纠治Ｅｂｓｔｅｉｎ畸形的手术效果。１９９７年７月至１９９８年１０月用Ｃａｒｐｅｎｔｉｅｒ环纠治Ｅｂｓｔｅｉｎ畸形１３例，年龄１２～４７岁，其中心功能Ⅱ级２例，Ⅲ级１１例，３例合并预激综合征（ＷＰＷ），术前作射频消融术。术中修补ＡＳＤ，房化心室折叠，然后置入Ｃａｒｐｅｎｔｉｅｒ环。结果本组无死亡，术后随访１个月至１年３个月，所有病人心功能都明显改善。心功能Ⅰ级１０例，Ⅱ级３例，４例心前区可闻及Ⅰ～Ⅱ级收缩期杂音，超声心动图显示４例有轻度三尖瓣关闭不全。Ｃａｒｐｅｎｔｉｅｒ环用于矫治Ｅｂｓｔｅｉｎ畸形，临床效果令人满意。Carpentier ring was used on 13 patients with Ebstein's anomaly for surgical repair from July,1997 to October,1998.The patients aged from 12 to 47 years (mean 16.7 years). Two of them were in heart function (NYHA) class Ⅱ and eleven were in class Ⅲ before the operation.Three had associated WPW(Wolff Parkinson White) syndrome on whom radio frequency ablation was done preoperctively. On each of the 13 Ebstein's anomaly cases,after closure of the ASD and plication of the atrialized right ventricle,Carpentier ring was inserted.There was none of operative death in the group of the patients.Follow-up of all 13 cases showed remarkable improvement of their heart function,10 patients in class Ⅰ and the other in class Ⅱ.
