中医药干预综合疗法治疗蝮蛇咬伤300例临床观察

摘　　要：目的观察中医药干预综合疗法治疗蝮蛇咬伤的临床疗效。方法将600例蝮蛇咬伤患者随机分为治疗组和对照组各300例。对照组运用抗蝮蛇毒血清、激素、抗生素、能量合剂等西医常规疗法,治疗组在西医常规疗法基础上运用隔蒜艾灸、外涂九味消肿拔毒散、口服717抗蝮蛇毒合剂治疗,于治疗前后对患者的局部症状(肿胀程度和范围、疼痛程度、瘀斑范围以及局部溃烂、局部麻木、局部出血情况)、全身体征(复视、瞳孔、眼睑下垂、张口困难、颈项及全身肌肉酸痛、烦躁、呼吸)进行评分,并观察临床疗效。结果治疗组总有效率为93.93%,对照组总有效率为73.22%,治疗组优于对照组(P<0.05);两组患者第3、第6天症状与体征积分均与本组治疗前比较差异有统计学意义,治疗组优于对照组(P<0.05);两组治疗后肿胀、疼痛积分均明显下降,并且治疗组优于对照组(P<0.05)。结论中医药干预综合疗法治疗蝮蛇咬伤有较好的临床效果。Objective To observe the curative effect of comprehensive TCM intervention on viper bites.Methods Six hundred patients with viper bites were randomized into treatment group and control group with 300 in each.The control group received the conventional treatment of western medicine,including anti-snakebite serum,hormone,antibiotics,and energy mixture,etc.,based on which,the treatment group received moxibustion with garlic insulation,application of Jiuwei Xiaozhong Badu San（Powder with nine ingredients in it for relieving swelling and removing poisonousness）,and 717 anti-snakebite mixture for oral taking. Before and after treatment,the local symptoms（severity and range of swelling,severity of pain,range of ecchymosis,and local ulceration,numbness,and bleeding） and systemic signs（diplopia,pupil,ptosis,difficulties in opening the mouth, neck and whole body muscle ache,restlessness,and breathing） were scored.Results The total effective rate of the treatment group was 93.93%,better than 73.22% of the control group（P〈0.05）.Compared with its own group before treatment,the scores of the symptoms and signs on the 3rd and 6th days of both groups after treatment were statistically different,the treatment group was better than the control group（P〈0.05）.The scores of swelling and pain of the two groups were significantly decreased after treatment and the treatment group was superior to the control group（P〈0.05）.Conclusion The effect of comprehensive TCM intervention on viper bites is good.
