额颞颧部的显微解剖及面神经保护

摘　　要：目的了解额颞颧部显微解剖结构。为临床额颞颧开颅术保护颞浅动脉主干及面神经颞支提供解剖依据。方法应用国人成人头颅防腐标本10例(20侧),模拟额颞颧入路对颞浅动脉和面神经颞支的走行及分布层次进行逐层解剖和分析。结果颞浅动脉主干位于耳屏前(8.1±2.4)mm。面神经额颞支位于耳屏前(14.9±2.1)mm,在颧弓上缘距离颞浅动脉起始部(6.8±3.1)mm。结论皮肤切口应位于颞浅动脉后紧邻耳屏前缘。在颧弓上缘切口深度应达到颧骨骨质表面,沿颧弓表面向前在颞深筋膜浅层下分离脂肪垫,取弧形切口并指向额骨颧突。Objective Understanding of frontotemporal zygomatic microscopic anatomy.provide anatomical basis for protection superficial temporal artery and facial nerve temporal branch in frontotemporal-transzygomatic craniotomy.Methods A microanatomical study was conducted in 10 cadaveric heads（20 sides） using frontotemporal-transzygomatic craniotomy.To the superficial temporal artery and the facial nerve temporal branch of the course and distribution levels of anatomy and analysis of drill.Results Superficial temporal artery is located in the tragus before（8.1±2.4）mm.Frontotemporal branch of the facial nerve at the tragus before（14.9±2.1）mm,the upper edge of the zygomatic arch from the Ministry of the superficial temporal artery（6.8±3.1）mm.Conclusions Skin incision should be located in the superficial temporal artery after,near the leading edge of the tragus.Incision in the deepth of the zygomatic arch should reach the surface of zygomatic bone,zygomatic arch,along the forward surface of the deep fascia in the superficial temporal fat pad under the separation,taking arc incision and point to the frontal zygomatic process.
