摘  要:目的:探讨血肿中的铁对皮瓣的毒性作用。方法:8头4~8月龄白色小型猪,在每头猪脊柱两侧各设计6个任意皮瓣,共96个。将皮瓣随机分配进入对照组、生理盐水组、血肿组、去铁胺组,每组24个皮瓣。对照组:皮瓣形成后无其他处理;生理盐水组:在皮瓣形成后向皮瓣下注入15ml生理盐水;血肿组:在皮瓣形成后向皮瓣下注入15ml自体血液;去铁胺组:在皮瓣形成后向皮瓣下注入15ml自体血液和270mg甲磺酸去铁胺。各组分别在术后24h、72h、120h取材,检测各组术后2h皮瓣的血流情况,检测皮瓣铁含量、丙二醛(MDA)含量、髓过氧化物酶(MPO)活性。结果:①四个组术后2h皮瓣血流灌注量无显著差别(P>0.05);②生理盐水组各项指标与对照组无显著差异(P>0.05);③血肿组术后皮瓣铁含量、MDA含量和MPO活性均高于对照组(P<0.01);④去铁胺组皮瓣铁含量和MDA含量低于血肿组(P<0.01),MPO活性与血肿组无显著差异(P>0.05)。结论:血肿中的铁能增强皮瓣内的脂质过氧化反应。Objective To explore the toxic effects of iron in hematoma on random skin flap.Methods Eight 4~8-month-aged white minipigs were selected.12 random skin flaps were designed on two side of vertebral column in each minipig.The total flaps were 96.The skin flaps were divided into control group,saline group,hematoma group and deferoxamine group randomly.The control group: there was no other treatment after flap formation;The saline group:15ml saline was infused into the capsules under the flaps.The hematoma group:15ml blood was infused into the capsules under the flaps.The deferoxamine group: 15ml blood and 270mg deferoxamine were infused into the capsules under the flaps.Seperately,pieces of skin flap tissue were taken from the skin flaps 24h,72h,120h postoperative.The blood flow of skin flaps were detected 2h postoperative.The contents of iron and malonaldehyde(MDA),the activity of myeloperoxidase(MPO) were detected.Results ①The blood flow of skin flaps which were detected 2h after theoperation had no significant difference among the three groups(P﹥0.05).②The indexes of saline had no significant difference with the control group(P﹥0.05).③Compared with the control group,the contents of iron and MDA and the activity of MPO in hematoma group increased(P﹤0.01).④Compared with the hematoma group,the contents of iron and MDA in deferoxamine group decreased(P﹤0.01).The activity of MPO had no significant difference between hematoma group and deferoxamine group(P﹥0.05).Conclusions Iron in hematoma can strengthen the lipid peroxidation in the skin flap.
