关节镜下治疗踝前外侧软组织撞击综合征的疗效

摘　　要：目的：探讨关节镜下微创治疗踝关节前外侧软组织撞击综合征的临床疗效。方法：2008年9月～2010年7月，对27例踝关节前外侧软组织撞击综合征患者，行关节镜下等离子射频刀清理术。结果：术后得到随访26例，失访1例。平均随访1．5年，24例随访患者踝关节疼痛肿胀消失、背伸跖屈活动度正常、功能恢复到伤前水平，3例踝关节背伸活动轻度受限，活动后肿胀疼痛。踝关节间隙无变窄。按照McGuire评分系统对患者术前、术后的情况进行评估，术后（90．6±11．9）分较术前（68．6．4±12．5）分明显提高，具有统计学意义（t=8．46，P〈0．05）。术后优良率88．5％，无1例感染、无血管神经损伤和术后复发。结论：运用关节镜下微创手术治疗踝关节前外侧软组织撞击综合征，操作简便，安全有效，创伤小，康复快。Objective ： To study the therapeutic effect of inimally invasive arthroscopic treatment of anterolateral ankle soft tissue impingement syndrome patients. Methods ： From September 2008 to July 2010,27 cases of anterolateral ankle soft tissue impingement syndrome were treated with arthroscopic debridement of plasma radio frequency knife. Results： Among the 27 cases,26 patients were followed up and 1 patient was lost. The average follow-up was 1.5 years. 24 patients reported that ankle pain and swelling disappeared, with normal dorsiflcxion plantar flexion activity, and the functions recovered to the level before injury. 3 patients presented with mild limitation in ankle dorsiflexion activity, swelling and pain after the activity, and no ankle joint space narrowing.According to McGuire scoring system, preoperative and postoperative assessment was conducted, and the postoperative condition （ 90.6± 11.9 ）points, compared with the preoperative condition （ 68.6 ± 12.5 ） points, was significantly increased, which bad statistical significance （ t = 8.46, P 〈0.05 ）. Good rate was 88.5% after operation, and there was no case of infection, neurovascular injury, or recurrence. Conclusion ： Minimally invasive surgery using arthroscopy for anterolateral ankle soft tissue impingement syndrome is simple, safe, effective and less invasive with a quick recovery.
