急性脑静脉性血栓形成磁共振功能成像的实验性研究

摘  要:目的探索脑静脉血栓形成(CVT)MR脑功能成像的方法,进一步探讨急性脑静脉性血栓形成的病理生理学机制。方法经颅上矢状窦近尾部结扎处注射血栓形成因子,建立一个稳定的大鼠急性脑静脉血栓形成的动物模型,于术后不同时间点连续进行磁共振功能成像,并与病理学结果相对照。结果 MRI显示病灶均限于上矢状窦结扎的范围内,且各时间点病灶范围大小不等。CVT在0.5 h,1 h,3 h,DWI显示的病变比T2WI敏感。6 h,12 h,48 h显示的病变范围两者间差异均无统计学意义(P>0.05)。各时间点PWI显示了和DWI,T2WI相吻合的灌注缺损区。CVT后缺血区ADC值在0.5 h下降到正常组织ADC值的(58.8±3.3)%,以后逐渐增加,48 h时达到正常组织ADC的(83.1±4.6)%。显微镜下改变与MRI结果具有较高的一致性。结论 MRI是研究脑血管疾病病理生理学机制的最好手段,MR功能成像技术可用于监测实验性静脉血栓形成病变发生发展的时间过程和表现。Objective To explore the method of functional magnetic resonance imaging (fMRI) in the the diagnosis of acute cerebral venous thrombosis (CVT) and to further study pathophysiologieal mechanisms of acute CVT through MR images and histological changes. Methods A rat model of aeule cerebral venous thrombosis was estabfished by injecting thmmbus formation factor 150 μl. fMRI was performed at different postoperative time points and comparison was made with pathological results. Results All visible lesions of MR exami- nation were restricted in the domain of SSS ligated, and the size of lesions varied at different time points. Diffusion-weighted imaging (DWI) was more sensitive to discover lesions than T2WI at 0.5 h, 1 h,3 h time point. At 6 h, 12 h,24 h,48 h time point,area of lesions on DWI and T2WI hart no difference (P 〉 0.05 ). At each time point, peffusion defect on perfusion-weighted imaging (PWI) was well correlated with lesions on DWI and T2WI. ADC values decreased to (58.8±3.3)% of normal at the 0.5 h time point,but the value gradually increased after that; and the value increased to (83.1 ±4.6)% of normal at the 48 h time point. There was a high degree of consistency between the MR and microscopic findings. Conclusion MRI is an optimal method of studying pathophysiological mechanisms of cerebral vascular diseases. fMRI could be used to monitor the temporal course and appearance of the genesis and development of experimental CVT.
