摘  要:目的 了解8 FSAFL型Sw artz长鞘在心房扑动消融中的作用。方法 左前斜45度影像定位下,在三尖瓣环5~6 点处划线消融三尖瓣环与下腔静脉开口之峡部。结果 11 例普通型心房扑动患者全部消融成功,随访4~18个月,1 例复发,再次消融获得成功。结论 在峡部消融中应用Swartz长鞘能使大头导管良好固定,并使峡部组织与大头接触密切,较易做到峡部线性消融,达到相当于完全性双向阻滞的满意消融效果。Objective To explore the effect of Swartz shealth of type SAFL during radiofrequency catheter ablation for common type atrial flutter. Method As viewed in left anterior oblique 45° projection,linear ablation was performed in the tricuspid annulus 5 to 6 o′clock under the support of Swartz shealth of type SAFL and bidirectional block in the isthmus between the tricuspid annulus and the inferior vena cava opening was achieved. Results In 13 cases of common type atrial flutter,all underwent successful ablation.After follow up of 4 to 18 months,atrial flutter recurred in one patient and ablated successfully by another procedure. Conclusion Under the support of Swartz shealth of type SAFL,ablation catheter electrode was stable and closely contact with the isthmus tissue.Real linear ablation in the isthmus was accomplished,so a complete bidirectional conduction block was guaranteed,the recurrence rate reduced as well as the fluroscopy time and the procedure time were shortened significantly.
