超声乳化白内障吸除折叠式人工晶体植入联合小梁切除术的临床疗效分析

摘　　要：[目的]评价慢性闭角青光眼合并白内障的患者采用超声乳化白内障吸除折叠式人工晶体植入术加同期联合小梁切除术的可行性、安全性及疗效。[方法]2007年1月～2010年1月我科治疗的89例慢性闭角青光眼合并白内障的患者,将接受超声乳化白内障吸除折叠式人工晶体植入联合小梁切除术的48例患者分为观察组,其余41例分为对照组,先接受小梁切除术,4周后进行超声乳化白内障吸除折叠式人工晶体植入。[结果]观察组术后3个月眼压正常者显著高于对照组。对照组术后并发虹膜损伤的例数显著高于观察组。而出现的浅前房及前房明显炎性反应并发症的例数差异无统计学意义。观察组与对照组术前、后视力情况及术后视力纠正情况对照结果显示,观察组和分期手术治疗的对照组术前、后视力对照结果差异有统计学意义。而两组术后视力纠正情况对照结果差异无统计学意义。[结论]超声乳化白内障吸除折叠式人工晶体植入联合小梁切除术治疗慢性闭角青光眼合并白内障临床效果较佳,且安全性高,给患者一个好的生活质量,值得临床推广。[Objective]To evaluate feasibility,safety and efficacy of Phacoemulsification-intraocular lens implantation combined with trabeculectomy in treatment of chronic closed Angle glaucoma with cataract patients.[Methods]From January 2007-January 2010,89 cases of chronic closed Angle glaucoma with cataract patients were collected,and accepted phacoemulsification-intraocular lens implantation combined with trabeculectomy 48 patients were divided into the observation group,the remaining 41 cases were divided into control group,accepted trabeculectomy,and accepted phacoemulsification foldable intraocular lens implantation after 4 weeks.[Results]The number of normal intraocular pressure in the observation group after three months was significantly higher than that of control group.The number of patients with damaged iris in the controls was significantly higher than that in the observation group.It was significantly higher than that of the observation group.But the number of shallow anterior chamber and anterior chamber inflammatory reaction of complications did not show significant difference with the controls.There was significant difference between the vision in the observation and control group before and after treatment.While there was no significant difference in the visual acuity situation in the two groups.[Conclusion]Phacoemulsification-intraocular lens implantation combined with trabeculectomy＇s clinical effect is better,and high safety,give patients a good quality of life and clinical promotion.
