摘 要:目的评价高血压合并腔隙性脑梗死患者与脑血管反应性的关系。方法 64例原发性高血压患者按是否合并腔隙性脑梗死分为2组,高血压合并腔隙性脑梗死组(腔梗组)31例,高血压无腔隙性脑梗死组(高血压组)33例,另选性别、年龄匹配的正常对照组(对照组)35例。用经颅彩色双功超声(TCCDS)检查静息状态下及屏气25s后大脑中动脉(MCA)的平均血流速度(Vmca)。通过计算高碳酸血症后Vmca对静息状态下的增加率(△Vmca)来评价脑血管反应性。结果腔梗组脑血管反应性低于高血压组和对照组并有统计学意义(P<0.05,P<0.01),高血压组脑血管反应性低于对照组并有统计学意义(P<0.05)。结论原发性高血压患者脑血管反应性减低,合并腔隙性脑梗死时,脑血管反应性进一步减低。Objective To evaluate the relationship between essential hypertension with lacunar infarction and cerebrovascular reactivity.Methods 64 patients with essential hypertension(eH) were examined.eH patients was divided into two groups,eH patients with lacunar infarction group(31cases) and eH patients without lacunar infarction group(33cases).We also examined 35 sex-and age-matched normal controls.The mean velocity of middle cerebral artery(Vmca) were detected using transcranial color-coded deplux sonography(TCCDS) at rest and after 25s of breath holding.The cerebrovascular reactivity was evaluated by calculating the increase percentage of Vmca(△Vmca).Results The cerebrovascular reactivity was significantly lower in lacunar infarction group than that in essential hypertension group(P0.05) and control group(P0.01),and it was also significantly lower in essential hypertension group than that in control group(P0.05).Conclusion The cerebrovascular reactivity decreased and the cerebrovascular reserve capacity was damaged among essential hypertension patients,and the damage was severer among the essential hypertension patients with lacunar infarction.
